Program Assessment & Agreement
All new Chelsea House program applicants are required to complete an assessment and read/sign the Good Samaritan Network program agreement. Both the assessment and agreement are linked below.

​Please complete the forms below (Program Application and Program Agreement) to apply for the program. The assessment tells us about you. The agreement tells you about the program. Please note that we are not able to mail forms to applicants.

Step Two:
Submit your completed assessment and agreement to our office for review.

Step Three:
The application will be reviewed by our advocacy team. A follow-up interview will be scheduled if necessary.
